Please use the form below to provide me with a brief overview of your financial situation, so that I can give you some basic advice about your case.
Before I can provide the review, you MUST read and accept the terms of the Consultation Agreement and BAPCPA Notices, which are also contained in the Initial Consultation Packet. I cannot provide you with any bankruptcy advice or assistance until you have reviewed those documents.
If you are not comfortable with providing information about your finances online, you can print out and complete the Initial Consultation Packet and fax the papers to us at
801-406-1027, or you can scan and email the complete documents to casereview@bkutah.com.
Some quick instructions:
- It is not necessary to enter dollar ($) signs.
- At this stage, it is not necessary that you give me exact dollar amounts for everything. But please do try to give me at least your best "guesstimate."
- Please provide household (not just individual) income and expenses.
- Please answer as many questions as you can. If something doesn't apply, you can leave it blank.
Your answers will give me a quick overview of your situation. My advice may change as I gather more detailed information about you, your case, and your financial situation. While the online case review can be useful as you begin to explore your options, in many cases it is more useful to visit with me in person or by telephone. Please feel free to request an appointment.
I will review the information you provide and respond by telephone or email as soon as possible, usually in much less than 48 business hours.
